How to obtain Birth and Death Certificates
To receive a copy of a standard or heirloom birth certificate or death certificate,
you must:
- You can either Order Online or by phone,.
- You must complete a birth or death certificate request.
- Submit a copy of your identification.
- Submit proof of your eligibility.
- Pay the non-refundable application fees.
- Submit your application to us.
