San Juan Basin Health Department Birth or death certificates may be obtained either through San Juan Basin Health Department or directly through the Colorado Department of Public Health & Environment.Obtaining a Birth CertificateBirth certificates are confidential and can only be given to blood relatives. Birth certificates can be released to:the mother the father - only if is listed on the original birth certificate maternal grandparents (mom's parents) paternal grandparents (father's parents only if he is on original birth certificate) the brother/sister (proof of relationship may be required) If you wish to get a birth certificate through San Juan Basin Health, you can download the form linked below, fill it out and bring it to our office with the proper identification (listed on the
form)
