                                  How to obtain Birth and Death Certificates

 

To receive a copy of a standard or heirloom birth certificate or death certificate,

you must:

  1. You can either Order Online or by phone,.
  2. You must complete a birth  or death certificate request.
  3. Submit a copy of your identification.
  4. Submit proof of your eligibility.
  5. Pay the non-refundable application fees.
  6. Submit your application to us.
